JACKSONVILLE, Fla. (Oct. 8, 2019) – Dee Powell, a nurse case manager at Naval Hospital Jacksonville’s Pediatrics Clinic, discusses care options with a patient. National Case Management Week is Oct. 13-19 and recognizes the dedication, compassion, and care outcomes achieved by case managers. Powell, a native of Jacksonville, Florida, says, “We assess, assist, and teach patients how to obtain what they need to meet their health care goals. We solve world problems one patient at a time. Just a grain of sand, but without those grains of sand there would be no beach. I’m proud to say I love my job.” Case managers provide collaborative clinical assessment, care coordination, patient education, counseling, case monitoring and clinical pathway management, discharge planning, resource management, and patient advocacy.
