High reliability organizations encourage everyone to be sensitive to their environment and have collective mindfulness. This requires everyone within the organization to consistently focus on processes that have potential to harm, recognize emerging problems earlier, and manage them more
decisively before they escalate.
Physician leaders can use this collective mindfulness concept while performing their different roles during patient safety meetings to help identify and prioritize needs, obtain resources and referrals, and to respond to patient and environment changes. We encourage you to consider how you can use this practice at your facility to bolster the role of the physician champion in patient safety meetings, moving it one step closer toward the goal of becoming a High Reliability Organization
