Reducing delays in specialty care and helping service members return to duty sooner are among the efforts that earned two Kenner Army Health Clinic nurse case managers recent People’s Choice Awards.
Corretta Custis and Melanie Stewart were recognized for their individual efforts to improve care coordination, reduce barriers and help patients receive the care they need.
Custis, a nurse case manager for the Active-Duty Clinic, was recognized for coordinating care for service members with complex medical conditions. Through proactive case management, she helped reduce time spent on medical profiles, support return-to-duty readiness and minimize delays in specialty care.
She also streamlined referrals, utilized military treatment facility resources when appropriate and incorporated virtual appointments to provide patients with greater flexibility and access to care.
Melanie Stewart, a nurse case manager for Pediatrics, was recognized for helping a patient receive specialty care months sooner. She collaborated with Naval Medical Center Portsmouth to coordinate an appointment with its developmental pediatrics clinic after identifying that the patient had previously received care there.
Her efforts resulted in the patient receiving an appointment approximately six months sooner than what was available in the civilian sector, significantly reducing the patient's wait for specialty care.
The efforts of Custis and Stewart demonstrate the impact nurse case managers can have on the patient experience. From supporting service member readiness to finding solutions that reduce lengthy waits for specialty care, both employees were recognized for helping patients get the care they need.
