Every day at Kenner Army Health Clinic, a team of Nurse Case Managers works quietly behind the scenes to ensure patients receive coordinated, compassionate, and continuous care. During National Case Management Week, Kenner celebrates these dedicated professionals whose teamwork and expertise strengthen the health and readiness of the soldiers, families, and retirees they serve.
Case managers are a vital part of the healthcare system. They help patients navigate complex care plans, coordinate between providers, and make sure the right support is in place at the right time. Their impact reaches across departments — improving outcomes, reducing fragmented care, and empowering patients to take an active role in their health.
For Itanya Milligan-Artis, Nurse Case Manager for the Family Medicine Clinic and Troop Medical Clinics, her passion for the field began decades ago. “The one-on-one time with patients in their home environment gave a better outlook on their needs,” she said.
In Behavioral Health, Geraldine Ervin shared that collaboration is what makes their team so effective. “We collaborate with each other and pull information from our past experiences,” she said. Her colleague, Sherri Ann Smith, emphasized the research and coordination behind every patient’s care. “Case Managers often have access to records to research a patient’s history which is essential for high quality care,” she said.
Corretta Custis, Nurse Case Manager with the Active-Duty Clinic, explained how their work directly supports readiness and overall wellness. “We advocate for our patients and educate them, so they can be active participants in their care and overall wellness,” she said.
For Melanie Stewart, who serves in Pediatrics and Family Medicine Clinic, the week is a meaningful reminder of why they do what they do. “Case Management Week is a time to celebrate service, kindness, and unwavering commitment,” she said.
At Kenner Army Health Clinic, those values come to life every day. The Case Management team’s dedication, compassion, and collaboration ensure that every patient receives the care, guidance, and advocacy needed to achieve their best possible health.
