The TRICARE active duty family member dental plan is expanding coverage to military families living in Canada.
Coverage in Canada is the same as for any U.S. location or other areas covered in the plan. Military sponsors must enroll family members before they receive dental services.
U.S. military family members in Canada may go to any licensed dentist of their choice. Normally, TRICARE pays dental claims to the patient. The patient is then responsible for paying the dentist. Claims will be based on allowed amounts set by the contractor. If the amount paid by the contractor is less than the amount charged by the Canadian dentist, the patient is responsible for the difference.
CHAMPUS officials said either the dentist or the patient may submit claims. Any standard claim form may be used. However, the form must include the sponsor's name and Social Security, number and indicate the patient has coverage in the TRICARE dental plan.
Until Jan. 31, sponsors must mail completed dental claims to one of two addresses, depending on where in Canada the dental care was received. Mail claims for care received in Alberta, British Columbia, Manitoba, the Northwest Territories, Saskatchewan and the Yukon Territory to:
Family Member Dental Plan
P.O. Box 269023
Sacramento, CA 95826-9023.
Mail claims for care received in Labrador, New Brunswick, Newfoundland, Nova Scotia, Ontario, Prince Edward Island and Quebec to:
Family Member Dental Plan
P.O. Box 9086
Farmington Hills, MI 48333-9086.
Beginning Feb. 1, a new contractor, United Concordia Companies, Inc., will operate the dental plan. Its mailing address for services furnished in Canada on or after Feb. 1 will be:
United Concordia Companies, Inc.
FMDP Claims Processing
P.O. Box 98220
Camp Hill, PA 17089-8220.
This will also include ongoing orthodontic treatment or scheduled appointments slated on or after Feb. 1. Written requests for claim forms and general information, and inquiries about claims after Feb. 1 must be sent to:
United Concordia Companies, Inc.
Customer Service P.O. Box 898218
Camp Hill, PA 17089-8218.
Officials said all claims payments are in U.S. dollars, with most claims processed within 21 days of receipt.
