On Sept. 8, American Forces Press Service published an article titled “Pakistan Tops List of Challenges, Gates Says” that was based on the previous evening's "Charlie Rose Show" on PBS. The article was not on the original AFPS schedule, and was assigned that morning based on an anecdotal report that the secretary of defense and chairman of the Joint Chiefs of Staff had appeared on the program.
The reporter assigned to write the article obtained a transcript of the program and wrote the story, failing to notice that the show was a compilation of Afghanistan-related interviews from the past year. The article was written as if the appearances by the secretary and the chairman were new, and the editors did not investigate the matter. The article was posted after normal hours Sept. 8, and was deleted the morning of Sept. 9 when the circumstances came to light.
