The procedures to outprocess from Ramstein can sometimes be overwhelming, but below is some information that may help alleviate that.
Exceptional Family Member Program
For service members who have received stateside assignments and have dependents enrolled in EFMP, the following documents are required.
• Air Force Form 1466, travel clearance paperwork for family. All dependents should be listed on this form. This form must be signed by the sponsor.
• Defense Department Form 2792. Required for each EFMP dependent traveling to a new assignment and/or on orders. A clearance statement in the electronic medical record can be used in place of the DD Form 2792.
• DD Form 2792-1. Required for each EFMP dependent from birth to age 21 traveling to a new assignment and/or on orders. Education forms are filled out by the school or daycare provider. If home-schooled, fill out the first page only.
• AF Form 1466-D. Dental clearances are NOT required for stateside assignments.
For service members with overseas assignments and EFMP dependents the following are required.
• AF Form 1466. Travel Clearance paperwork is required for all family members. All dependents should be listed on this form and it must be signed by the sponsor.
• DD Form 2792. Each dependent is required to have an overseas physical from their physician. The physician will decide whether the DD form 2792 is necessary to complete. If the physician does not provide a DD Form 2792, a copy of the medical note can be submitted to the EFMP office.
• DD Form 2792-1. Required for each dependent from birth to age 21 traveling to new assignment and/or on orders. Education forms are filled out by the school or daycare provider. If homeschooled, fill out the first page only.
• AF Form 1466-D. Dental Clearance is required for each dependent age two and older traveling to a new assignment and/or on orders.
Service members with stateside assignments, whose dependents are not registered with EFMP, need to fill out AF Form 4380, Air Force Special Needs Screener.
If service members answer “yes” to any questions on the screener, they must bring the completed form to EFMP office at building 2114, room 147 or contact EFMP at DSN 479-2375 or 06371-46-2375.
Military members without dependents don’t need to outprocess with the EFMP office.
Tricare
To outprocess, military members must fill out a portability form seven days prior to their fly-out date and take it to the Tricare office or email it to 86mdss.outpatientrecords@us.af.mil. The portability form can be found at the Tricare office or online.
Dental
Flyers and Personnel Reliability Program members will hand-carry their records, but must first go to the dental office a day before their final out. They should bring two copies of their orders in order to receive physical copies of their dental records.
“Military members that are not flyers or part of the PRP can be cleared through military records at the Ramstein Medical Clinic, once they have their dental clearance exam,” said Tech. Sgt. Allison Ali, 86th Dental Squadron, records and reception noncommissioned officer in charge.
The 86th Dental Clinic is open from 7:00 a.m. - 4:30 p.m., Monday through Friday, and is located in building 301.
The Exceptional Family Member Program office is open from 7:30am-4:30pm Monday through Friday and is located in the 86th Medical Clinic, building 2114.
The Tricare office is open from 7:30 a.m-4:30pm, Monday through Friday and is located in the 86th Medical Clinic, building 2114.
