    FORT HOOD, Texas - Fort Hood officials have released the name of a soldier who died from a terminal illness July 12 at Scott & White Hospital, Temple, Texas.

    Spc. Charles L. Simmons, 34, whose home of record is listed as Waco, Texas, entered active-duty service in January 2007 as a cavalry scout. He was assigned to 504th Military Intelligence Battalion since October 2010.

    Simmons deployed in support of Operation Enduring Freedom from July 2009 to July 2010.

    Simmons’ awards and decorations include the Army Achievement Medal, Army Good Conduct Medal, National Defense Service Medal, Afghanistan Campaign Medal with combat service stars, Global War on Terrorism Service Medal, Army Service Ribbon, NATO Medal, Combat Action Badge, Driver and Mechanic Badge with wheeled vehicle skill tab.
