The current officer evaluation report form, Department of the Army Form 67- 9, will be replaced by DA Form 67-10, April 1, 2014. The change is a part of the revision to the Officer Evaluation Reporting system. Soldiers with the Army Human Resources Command visited Fort Hood officers Feb. 24-28 to discuss the new changes.
