Winn Army Community Hospital will host a Health Fair to complete all examinations and paperwork for the Georgia Public School Health Screening, DoD School Physicals, and the Sports Participation Examinations, June 26. Included as part of this are the following: dental hygiene evaluation, vision testing, hearing evaluation, nutrition optimization, scoliosis screen, immunizations, and the COVID vaccine for children 12 and over. Events are 8 a.m., 11 a.m., and 2 p.m. on June 26, July 31, and Aug. 7. Call the appointment line at (912) 435-6633 to reserve your appointment. Check-in for appointments will be in the main lobby of Winn Army Community Hospital - near the coffee shop.
Forms for the Georgia School Health Screening, DOD School Health Physicals and Georgia Sports Participation Physicals will be provided at the event, but are also enclosed if you would like to pre-fill and bring with you. If additional forms need to be completed for other programs (i.e. Girl/Boy Scouts, youth soccer, private schools), please bring those forms with the child’s information completed on the day of your appointment. *Please note that we will not be offering prescription refills, EFMP paperwork, or other health referrals/exams at this event.
