Naval Hospital Bremerton (NHB) is helping military families get a healthy head start on ensuring their school age children are back to school/sport physical ready for the upcoming school year.
NHB Family Practice and Pediatric clinics – as well as Branch Health Clinic Bangor - will be holding the annual Back to School and Sports Physical Rodeo for eligible patients on Wednesday, August 7 from 4:20 p.m. to 6 p.m., and Saturday, August 10, from 8 a.m. to 10:40 a.m. for all eligible beneficiaries.
Children under the age of 18 must be accompanied by a parent or legal guardian with the child’s dependent identification card. It is also recommended to provide the school district’s specific sport physical form for the time the student will be attending for the upcoming school year.
Immunizations will be available as needed for patients with scheduled appointments.
Patients are request to arrive approximately 10 minutes prior to the scheduled appointment.
Appointments may be booked via the Puget Sound Military Appointment Center at 1-800-404-4506 during Monday-Friday 6 a.m. to 8 p.m. and weekends at 7 a.m. to 3:30 p.m. No walk-ins will be taken.
