    Public Health Center Launches Online Chronic Condition Library

    PORTSMOUTH, Va. – The Navy and Marine Corps Public Health Center’s (NMCPHC) health promotion and wellness (HPW) department has launched its first chronic condition-specific online resource library – the Chronic Condition Health Education Toolbox, Mar. 15, 2018.

    The Chronic Condition Health Education Toolbox provides patients, health care providers and medical staff with a one-stop shop of resources and information on cholesterol, diabetes, healthy weight, high blood pressure and tobacco cessation.

    “Sometimes there is just too much [health] information to sift through, which can be overwhelming and people are always in a hurry, so we need to make it easy for them,” said U.S. Navy Bureau of Medicine and Surgery (BUMED) Branch Head for Care Management, Cmdr. Kendra A. Manning.

    Resources within the toolbox are designed to deliver credible and relevant health information to patients and providers. The resources include guides, fact sheets, activity logs and videos developed by NMCPHC and trusted partner organizations, such as the Centers for Disease Control and Prevention, National Institutes of Health and U.S. Department of Health and Human Services.

    “Having easily-accessible and trusted resources will hopefully increase patient engagement and participation in their health care,” said Manning. “We want to see patients becoming advocates for themselves to improve their health and quality of life. For providers, having a central place makes their lives much easier because they do not have a lot of time to search and find the information they need.”

    Featuring a user-friendly design, the Chronic Condition Health Education Toolbox is organized into two sections – one for patients and another for health care providers and medical staff. It also provides quick downloads for PDFs and an easily accessible bookmarking feature.

    “Having a consolidated ‘go-to’ site provides an easy-to-use, efficient way for providers and patients to find information,” said NMCPHC Population Health Program Coordinator, Karen Jones. “The same is true for patients using the Toolbox. Active duty Sailors and Marines, along with their families, can find information housed in the Toolbox to educate themselves about their chronic health condition.”

    The ultimate goal for the Chronic Condition Health Education Toolbox is to improve independent health education and health education delivered by providers.

    “Those of us who have been involved in chronic condition prevention and management courses over the years know just how special the rewards can be,” said NMCPHC Public Health Educator and chronic condition subject matter expert, Jenni Osborne. “The best reward is watching a patient realize they do have control over some aspects of chronic conditions and have a role in increasing their chances of long-term good quality of life. Staying active, watching grandkids graduate, and living a normal life well into the future.”

    NMCPHC encourages all users to submit feedback and share resources by accessing the “Suggestion Corner” located in the Provider and Staff Resources section. Suggested submissions are vetted by NMCPHC subject matter experts and considered for inclusion.
