FORT HOOD, Texas - Carl R. Darnall Army Medical Center will hold this year's annual Sports Physical Clinics July 27 and September 7 from 8 a.m. to 3 p.m. at the Thomas Moore Health Clinic at Fort Hood.
Only Tricare Prime beneficiaries ages 4 - 18 years enrolled to CRDAMC clinics are eligible to participate in the Sports Physical Clinics. All participants are required to make an appointment prior to the event. Walk-ins will be redirected to their primary care clinics. Appointments are anticipated to be approximately two hours long from beginning to end.
Routine childhood immunizations will not be offered during the Sports Physical Clinics this year. Parents should contact their primary care clinics to receive information on immunizations.
Children with appointments must be accompanied by a parent or guardian and should be dressed in comfortable gym clothing and shoes. Those over the age of 10 must have a DEERS identification card in their name. Parents or guardians of children under the age of 10 will also be required to provide a DEERS identification card.
Additionally, participants need to bring a copy of their school's sports physical form and their immunization record.
The Killeen Parks and Recreation Department does not require sports physicals for participation. Killeen and Copperas Cove Independent School Districts only require sports physicals for students who are entering 7th, 9th or 11th grade unless there has been a change in the student's medical condition since their last examination. Fort Hood Child and Youth Services require physicals for children of all ages in order to participate in activities.
